You heal best at home! Christian Care Communities offers seniors the ability to heal at home with Christian Care At Home.  Providing a full-range of in-home care services, Christian Care At Home serves older adults in 10 west central Kentucky counties, seven days a week.


This position serves Logan and Warren Counties.


Our services include:

  • Skilled nursing care
  • Physical, occupational and speech therapy
  • Medical social services
  • Home health aides
  • Nutritional management and education
  • Durable medical equipment(referral/coordination)

The Medical Social Worker is responsible for evaluating patients by performing on-site clinical and financial assessments to determine their eligibility for medical and non-medical resources  related to their condition, discuss eligibility for assistance programs, and provide a description of community resources available as they apply to their situation by working with the patient and their family, while maintaining compliance with all applicable laws, regulatory and organizational standards.  


Required Education:                                       Master’s Degree in Social Work

Required Experience                                       1 year Social work experience in health care setting

Preferred Experience:                                     3 years experience in health care setting

Required License and/or Certification:           Licensed as Social Worker, Commonwealth of Kentucky


Business travel is a requirement of position; proof of valid driver’s license with a clean driving record

If personal vehicle is used to transport self or others; proof of insurance of $100,000 single limit of liability
