Responsibilities

Job Responsibilites for a Memory Care & Social Services Director:

 

The Memory Care & Social Services Director is responsible for championing the Best Friends approach to providing care for residents with cognitive issues, as well as staff training on the approach. This position additionally designs and implements programming for the Memory Care residents as well as works with families as Social Service issues arise. All responsibilities are performed while maintaining compliance with all applicable laws, regulatory and organizational standards.

 

  • Develops and conducts daily programming for Memory Care residents, and Assisted Care when needed.
  •  Champions Best Friends approach through continuous staff education.

  •  Coordinates Resident Life Stories.
  •  Educates and supports staff on resident behaviors and memory care needs.
  •  Completes and updates electronic medical records for residents.
  •  Identifies the need for, and obtains resources from, community social, health and welfare agencies to meet the needs of the resident.
  •  Provides informal counseling to uncover any problems which might be interfering with the residents’ socialization and participation in activities.
  •  Provides residents with opportunities to vocalize problems or issues they may have.
  •  Finds solutions to issues concerning the resident, family and facility.
  •  Communicates all grievances to the administrator.
  •  Attends Care Plan conferences and advises families with regard to care of resident; provides a better understanding of the residents’ issues and affords the relatives a channel of communication with the community.
  •  Facilitates monthly Resident Council Meetings.
  •  Assists in coordination of volunteer services within the community.
  •  Plans and designs monthly activity calendars.
  •  Assists with events within the community as needed.
  •  Assists in transporting residents to locations outside the building when needed.

 

Qualifications

Memory Care & Social Services Director Work Hrs./ Shifts:

  • Full Time / AM Shift

What will you need as a Memory Care & Social Services Director ?

  • Bachelor’s Degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology
  • Required Experience: 2 years’ experience in a social or recreational program setting
  • Preferred Experience: Skilled nursing facility knowledge/long term care background
  • Required Certification or Licensures: Commercial Driver’s License (CDL) License Class C with passenger endorsement, or ability to obtain within 90 days of hire
